Each QAC has a Census representative available to answer questions about the Census, and most are equipped with actual forms in English and Spanish that residents can pick up. (The hours listed are when the representative will be available; forms are available anytime during the sites normal operating hours).
BCs do not have a representative, but you can pick up a form.
*NO ID is required to get a form and residents can pick up multiples (i.e. for neighbors, other family members, etc.)
*Remember, only one questionnaire per residence (For example, if you fill out a questionnaire from a QAC/BC and then later get your questionnaire in the mail, you do not need to fill out both. But if you do, only one will count. Forms sent to homes are bar coded with an address, but on forms that you pick up from a QAC/BC you will be required to write your address. This is only to identify which residence the questionnaire pertains to and avoid duplication). |
